It’s World Stroke Awareness Day, and Menopause is associated with an increase in multiple stroke risk factors. Some aspects of women’s lives can increase our risk of a stroke, like the contraceptive pill, HRT, pregnancy and having migraines. But for most women, taking care of your health and managing your risk factors will help you avoid a stroke.
Stroke is the 4th leading cause of death and a major cause of disability. The risk of stroke increases with age. Although overall age-adjusted stroke risk is higher in men than women, more strokes occur in women because of their longer life expectancy combined with very high stroke rates in the oldest age groups. Women, therefore, account for 60% of all stroke events!
Cohort studies of healthy women moving through the menopausal transition have shown an increase in abdominal obesity, an increase in triglycerides, total cholesterol and LDL cholesterol, a decrease in HDL cholesterol, increased fasting glucose and other measures of insulin resistance, increased BMI, and increased blood pressure.
In addition, women who go through early menopause have an increase in evidence of subclinical vascular disease. The risk factors are likely due to the androgen excess and oestrogen decline which may explain the doubling of stroke risk in the 10 years after menopause.
Although women have a lower risk of stroke during middle age, the menopausal transition is a time when many women develop cardiovascular risk factors. In addition, during the 10 years after menopause, the risk of stroke roughly doubles in women. Endogenous oestrogen levels decline by 60% during the menopausal transition, leading to a relative androgen excess, which could contribute to the increased cardiovascular risk factors in women. Earlier onset of menopause may influence the risk of stroke, but the data are not clear.
HRT AND STROKES
Hormone replacement therapy Hormone replacement therapy (HRT) tops up your levels of hormones to control menopausal symptoms such as hot flushes, vaginal dryness and mood swings. There are different types of HRT which use oestrogen and progesterone in different combinations or alone. HRT tablets containing oestrogen cause a small increase in your risk of stroke. However, if you use a patch or a gel form of HRT with oestrogen, this does not increase your risk of clots or stroke.
If you have a family history of blood clots, you may need some extra checks before receiving HRT. Like the combined contraceptive pill, the increase in risk from taking HRT tablets is very small, particularly if you are under 60. So provided you are in a low-risk group, or your risk factors are well managed, your overall risk is still very small.
It's vital to have regular Blood Pressure checks with your Doctor, and I recommend women have a home Blood Monitor machine, especially those with a family history or on blood pressure medication.
